Multi-institutional Phase II \t\t\ttrials are examining 60 Gy in 3 fractions delivered by linac-based, \t\t\t\u06f3D-conformal IMRT. HiArt Helical TomoTherapy is a treatment unit that \t\t\tdelivers co-planar helical IMRT that is capable of image-guided SBRT. \t\t\tWe hypothesized that the maximum tolerable dose (MTD) delivered to a \t\t\tlesion by Helical TomoTherapy-based SBRT could be predicted based on \t\t\tthe planning target volume (PTV) and liver volume. METHODS AND \t\t\tMATERIALS: To test this, we performed inverse treatment planning and \t\t\tanalyzed the dosimetry for multiple hypothetical liver gross tumor \t\t\tvolumes (GTV) with conventional PTV expansions. Inverse planning was \t\t\tcarried out to find the maximum tolerated SBRT dose up to 60 Gy to be \t\t\tdelivered in 3 fractions based on the dose constraint that 700 cc of \t\t\tnormal liver would receive less than 15 Gy. RESULTS: Regression \t\t\tanalysis indicated a linear relationship between the MTD, the PTV and \t\t\tthe liver volume, supporting our hypothesis. A predictive equation was \t\t\tgenerated, which was found to have an accuracy of +\/-3 Gy. In addition, \t\t\tdose constraints based on proximity to other normal tissues were \t\t\ttested.
